RESOLUTION NO. ______________

A RESOLUTION OF THE TOWNSHIP BOARD OF SUGAR CREEK TOWNSHIP
REGARDING THE ADMINISTRATION OF TOWNSHIP ASSISTANCE
WHEREAS, pursuant to Indiana Code § 12-20-5.5-1, the Trustee of Sugar Creek
Township (the “Trustee”) has completed an annual review of the emergency assistance
eligibility standards (the “Current Standards”) for the administration of Township Assistance
(the “Township Assistance”) in Sugar Creek Township, Hancock County, Indiana (the
“Township”) and proposed Modifications (as defined herein) to the Current Standards to reflect
changes in the cost of basic necessities in the Township and changes in the law; and
WHEREAS, the Current Standards and process for Township Assistance are available
at http://www.sugarcreektwp.com/township-assistance, subject to the following proposed
modifications: (i) there shall be a total cap per household of not to exceed $1,000 annually; and
(ii) a limit of one receipt of funds per household annually
(collectively, the “Modifications” and
with the Current Standards, the “Standards”).
WHEREAS, the Township Board (the “Board”) of the Township, has reviewed the
proposed updated Standards and desires to approve the Standards to implement the Township
Assistance in conformance to Indiana Code § 12-20-5.5-1.
NOW, THEREFORE, BE IT RESOLVED BY THE TOWNSHIP BOARD OF
SUGAR CREEK TOWNSHIP THAT:


1. Pursuant to Indiana Code § 12-20-5.5-1, the Standards and administration of the
Township Assistance shall be governed by Indiana Code § 12-20 et seq. and all related
regulations, requirements and standards.


2. The Trustee has proposed the Standards, and the Board hereby adopts the
Standards.


3. Following adoption of this Resolution, the Standards shall be: (i) filed with the
Board of Commissioners of Hancock County; (ii) published in a single written document,
including addenda attached to the document; and (iii) posted in a place prominently visible to the public in all offices of the Trustee where the Township Assistance applications are taken or
processed.
4. This Resolution is effective immediately upon passage.

Eligibility for assistance is determined on a case by case basis. The steps for applying for assistance are as follows:

Step 1: 

Download and fill out the “Verification of Eligibility for State or Local Public Benefit” form below. This form should be printed and filled out completely.

Step 2: 

Dowload and fill out the “Employment Verification Sheet for Township Assitance 2023” form below. Note, this form must be filled out for each working age (18) person in the residence. This form should be printed and filled out completely.

Step 4: 

Once you have completed the three above steps in their entirety, you may contact Deputy Trustee Tucker at (317) 861-6148 to schedule an in office interview to review all documents included in your application for Township Assistance. Interviews are scheduled on Wednesdays of each week with Deputy Trustee Tucker. Once your assistance application has been reviewed, you will be notified via phone or email if you have been approved. An additional appointment may be scheduled to finalize all paperwork

If you are denied township assistance, you can appeal that denial with the Hancock County Commissioners Office

Required Documents for Township Assistance are listed below:

1. All forms above in steps 1, 2 and 3 must be completed in their entirety.  If you cannot complete the online application contact the trustee's office and a copy will be emailed to you. 

2. Proof of Residency (Valid driver's license, Indiana ID Card, bill or bank statement).

3. Social Security cards for all members of the household. If Social Security card is lost, please submit proof or renewal request.

4. Copy of birth certificates of all children in the household.

5. Proof of all income for the last 30 days for every person over the age of 18 and not attending school (pay stub, unemployment benefits, state assistance, child support, disability, SNAP benefits etc).  

6. Bank Statements (checking and savings) showing transactions of at least the last 60 days  (including transactions between your last statement and appointment date).

7. Current lease or most recent mortgage statement.

8. Copies of bills for which assistance is requested such as a disconnect or eviction notice. 

9. Medical information (If Applicable) : Doctor's statement pertaining to the illness or Disability Application Process.
